Inform us if you have any of the following
- Have been diagnosed positive for the COVID-19 Virus at any time
- Are currently awaiting the results of a COVID-19 test
- fever, dry cough, runny nose, sore throat
- any shortness of breath
- experienced headaches, fatigue, or weakness
- lost sense of taste and/or smell
- have traveled to a foreign country or throughout the United States within the last 14 days
